Abstract

Hyperosmolar Hyperglycemic State can be defined as a clinical condition characterized by a significant increase in glucose levels, hyperosmolarity, dehydration and little or null ketosis levels. Among the most frequent triggers are infections and pneumonia, followed by urinary tract infections and sepsis. The nursing process that is presented shows an intervention plan in nursing to an individualized patient, which is based on a real clinical case with Hyperglycemic Hyperosmolar state with Acute Renal Insufficiency complication, hospitalized in the Intensive Care Unit in a hospital high specialty, using the integral assessment of the nursing process in its five phases, based on the 11 functional patterns of Marjory Gordón, counting on the successive phases of the Nursing Care Process and establishing the necessary nursing care based on the first three Nursing diagnoses: excess fluid volume, deterioration of gas exchange and decrease in cardiac output. The intervention plans that were developed in this work were: fluid monitoring, acid-base balance management, hemodialysis treatment, respiratory monitoring, invasive mechanical ventilation management and cardiac care. The nursing care plans that were implemented in this patient were very useful, they were given specific and priority care, the continuity of the process was maintained, allowing a significant advance in the recovery of the patient and the discharge of the Intensive Care Unit service.
